JOB PURPOSE: Responsible to provide remote technical support and training for individual patients utilizing our online proprietary platform and various digital medical devices to measure daily vital signs.
MUST HAVES: Must be a Kentucky Resident
Experience Specifically Required:
- At least two or more years of previous experience working in a clinical setting
- Understands remote patient monitoring, telehealth, care/case management, chronic disease
- Associate degree in Nursing with two years of experience with LPN (Licensed Practical Nurse) ORa Certified Medical Assistant with at least three years of experience, OR the equivalent combination of education and experience.
- Certificates, Licenses, Registrations, Professional Designations:
- Licensed Practical Nurse (LPN/LVN), Registered Nurse (RN), ORCertified Medical Assistant (CMA) required
YOU WILL BE RESPONSIBLE TO:
- Manage an active caseload of patients who have subscribed to our RPM+ (Remote Patient Monitoring) platform or are engaged in other CW telehealth programs; monitor and manage patient chronic disease.
- Onboard and assist patients in the set-up and operation of our “MyWellness Connect” mobile app, and FDA-approved, Bluetooth-enabled devices to monitor specific health conditions.
- Utilize our proprietary telehealth system and dashboard to interact with and monitor patients.
- Work with patients who have Prediabetes/Diabetes, Heart Disease, High Blood Pressure, COPD, Sleep Apnea, Asthma, Obesity, COVID-19, among other illnesses.
- Review and evaluate patient behavior, telereadings; provides education and proactive health and wellness support as needed for individuals suffering from one or more serious medical conditions.
- Communicate with patients daily via text, phone or video calls regarding patient disease state; engage in patient education, medication adherence, and more as assigned.
- Utilize our proprietary EHR (electronic health record) to track all patient data and information to communicate both with patients and co-workers; schedule patient appointments and manage schedules as required.
- Sets personal tasks and calendar reminders to respond to various patient-specific issues; coordinates schedule with other Clinical Coaches for coverage when needed.
- Performs all duties on an agreed-upon schedule as assigned by the supervisor.
- Participates in the company’s online patient community; moderate patient interactions with other patients.
- Participate in ongoing, internal, and external training to improve health coaching skills.
